We have an active and supportive patient group who meet every year. We welcome new patient members to join the patient participation group. If you are interested in joining the group please complete this form or give your details to Reception and we will contact you with further information. In event when PPG already has 25 members, candidate will be given the option of being placed on a waiting list to be invited to join the group in due course.
