Practice Boundaries

Please see map below. Roads within the dark grey border and not on the line make up the Practice area and within the light grey outer border is the outer boundary. If you are already registered with us and move into the outer boundary (light grey border) you can stay registered with the Practice.

We are currently accepting new patients onto our practice list. We aim to offer high quality of service to all of our patients. Patients requesting to join the practice are requested to:

Come between 11am and 4pm to complete a registration form and new patient questionnaire (please find them below)
Bring proof of address and photographic identification, such as a passport or drivers licence (if available)

If you wish to register online with us please complete the below Pre-Registration form and a Health Questionnaire form.

Please note both forms are necessary to complete your registration. Please also send a copy of your ID and proof of address if available.

Once you have completed the above forms you can either bring them to the surgery or you can send your completed registrations to enfccg.winchmore.admin@nhs.net.

Please note once we have received the forms, the registration can take up to 10 working days to process.

Please ensure that you have enough medication (at least one month) to last until your registration has been processed.
