Sickness Certificates are issued after 7 days of self-certification. The self-certicates should be provided by your employer. To enable a doctor to issue a certificate you must have been seen by the doctor for the illness you are away from work sick.
Sickness certificates can only be given if you have seen a doctor or spoken to a doctor for that complaint. For repeat sickness certificates for longstanding illnesses you must have been seen by the doctor signing the certificate within the last month.
Private certificates can be issued after 7 days of self-certification for which a fee is payable. Fees chargeable will be displayed at the reception desk.
Completion of medical insurance claim forms, and other non-NHS work, all attract a fee. Fees chargeable will be displayed at the reception desk. Forms that need completing by a Doctor may take 2- 4 weeks to complete.
All travel vaccines can be arranged at the surgery. If you are planning to go abroad it is advisable as part of your travel preparation to make sure you have appropriate vaccinations.
For some vaccines, an administration fee is payable and relevant prices can be found at the reception desk. Some vaccines are costly and please note we do not take credit cards, we only accept cash or a cheque with guarantee card, so be prepared to save you coming back.
Please download, print and fill out the Travel Risk Assessment Form and bring it to the surgery. The Nurse will look at this within 7 days and contact you to discuss or make an appointment. Please enquire about your travel vaccines at least 8 weeks before your intended date to travel to allow sufficient time for the vaccines to take effect.

We are very pleased to offer a new service to our patients*. Our existing nurse, Julie Crouch, is a qualified Specialist Nurse Practitioner and now provides Minor Illness / Triage consultations. You will be offered an appointment for the following conditions / problems:

Please call surgery on 020 3850 5000 after 11am to get your test results if you are unable to view them using NHS app.
We offer home visits to our housebound patients that are not able to come into the surgery. We request all home visit requests to be put in before 11am. The doctor will call the patient to assess whether or not they require a physical assessment. If after the assessment the doctor feels that you require a visit, one of the GPs will come out and carry out the consultation. It is important that if you are housebound and you require a home visit it is important to call the surgery as soon as possible.
